Where I live the only vehicle for which you do not need insurance and registration is bicycle. Even for bicycle you must have a driver's license.
For anything with an engine, regardless of displacement and power, you must have insurance, license plate, driver's license...
You can drive 50 cc scooter with license for a car, but for anything above 50 cc you must pass exam for bikes (theoretical part, 15 hours of driving, practical exam on proving ground, practical exam on the road).
The same is for quads.
All vehicles are inspected every 12 months when you have to prolong also insurance and registration.
You must wear a helmet when driving any bike, quad or bicycle.
